What is different about Ukrainian documents
Names are transliterated exactly as in your закордонний паспорт — Oleksandr, Yuliia, Kyiv — and never by a Russian-style rule unless your passport uses it. Tell us the spelling once; it is used on every page and on the certificate.
Patronymics (по батькові) are kept and never turned into a middle name; feminine surname endings stay as written, with a translator’s note where a US form needs one family name.
Institutions are rendered with their standard English names and the original in brackets where it matters: «ДРАЦС / РАЦС / РАГС» → “Civil Registration Office”, «ЦНАП» → “Administrative Services Center”, «МВС» → “Ministry of Internal Affairs”, «Міносвіти» → “Ministry of Education and Science”. Grades «відмінно / добре / задовільно» → excellent / good / satisfactory, with the 5- or 12-point scale explained once.
Every stamp, seal, wet signature and apostille is reproduced in the same position — [round seal: Shevchenkivskyi District Department of Civil Registration, Kyiv]. Dates stay in the original order with the month written out.

Which spelling of my name will you use?
